Honestly, for me the thing that makes it different is the added level of intimacy or humiliation. It’s your underwear and someone else has a hold it, so if you’re wearing underwear that’s “embarrassing” (tighty whities, thongs, or panties, for example) then that’s an added layer. Even if you know they’ll see them ahead of time, there’s still that little bit of “oh noooooo!”
On top of that, then there’s the fact that you don’t REALLY know what’ll happen with the wedgie. Someone else is in control of it and there’s no way to fully know when they’ll add pressure to your anus or your genitals. So that’s an added excitement as well!
